Find out morePre-admission is an important part of your day hospital care. Your doctor will explain your procedure and complete the consent forms with you. If you have any questions about your planned day surgery admission, the completion of forms, cost or your health insurance status, please contact our team who will be happy to assist you.
To ensure we can confirm your admission, health insurance and other details/arrangements please complete our online pre-admission form at least 5 days before your admission.
If you would prefer not to complete the admission form online, you can email or fax us the paper forms usually provided to you by your surgeon. If you do not have any paper forms please contact us prior to your admission date.
Please notify us if your weight is over 150kg prior to your admission as we will need to organise a pre-admission assessment.
If you have private health insurance please contact your health fund to check your level of cover for hospital admission.
Please ask these 5 questions:
After a general anaesthetic you must have an escort to accompany you home, and a responsible person must stay with you overnight after your procedure. If you do not have an escort, or overnight carer, then for your safety we cannot admit you to the hospital.
For children under 10, two responsible adults must be present in the vehicle, a driver and a carer.
If you can’t arrange an escort please contact our admissions staff who can put you in touch with a nursing agency. The agency can arrange for someone to act as your escort. This will incur a charge payable by you directly to them.
If you can’t speak or understand English well, and do not have someone who can speak English to accompany you to the hospital, then please ring the NSW Health Care Interpreter Service.

The service is free but you must arrange it before your admission day.
If your procedure is subject to Work Cover or a Third Party Claim, please send full details of the claim to the hospital, including a letter from your insurance company accepting liability for your admission to hospital for the procedure.
Unless your doctor gives you specific instructions
One or two days prior to admission a staff member will contact you to verify your admission, fasting time and confirm any hospital costs to be paid on admission.
